Big Baby follows Adam Lewis, a horror screenwriter plagued by online criticism and creative frustration. Adam becomes fixated on a disturbing figure that begins appearing in his dreams: a towering, ruthless killer wearing a grotesque baby mask. Adam dubs the character Big Baby and pours these visions of the killer into a new screenplay. The boundary between his imagination and reality starts to fracture, and the presence of Big Baby feels increasingly real.

When violent events from Adam's script begin manifesting in the real world, he is forced to confront the possibility that his creation has taken on a life of its own. Adam's fixation with Big Baby frightens and alienates his longtime girlfriend Kate but he is convinced that the only way to keep her safe is to follow through with Big Baby's bidding for bloody revenge on his childhood tormentors. Paranoia, guilt, and obsession tighten their grip as Adam spirals into a nightmare where inspiration becomes a curse and authorship comes at a deadly price.
